Who Qualifies for Our Services?
To be eligible to receive care at Community Helping Hands Clinic, Inc., an applicant must meet the following qualifications:

To Schedule an Eligibility Appointment
Currently, eligibility appointments are offered only on Wednesdays. To schedule an appointment call 706.865.1223.
Please bring these documents to your Eligibility appointment:
• Proof of residence such as a utility bill
• Proof of income (pay stub, W-2 form, IRS return, etc)
• Application
• Government-issued form of ID (Driver’s license)
Our Services are for Chronic & Sub-Acute Health Issues
Our services include laboratory monitoring, electrocardiograms, as well as some imaging procedures for:
- COPD/Asthma
- Hypertension
- Hyperlipidemia
- Diabetes Mellitus
- Hyper/Hypothyroidism
- Cardiovascular Disease
Community Helping Hands Clinic, Inc. will NOT provide:
- Urgent/Emergent Care Services
- HIV/AIDS Managements
- Chronic Pain Management (Requiring use of controlled substances)
- Mental Health Services
- Reproductive Health Services For Women
- Care normally provided by specialists
We also provide access to limited dental and eye care.
Services are provided by health care professionals and dedicated members of the community. While laboratory samples can be collected on site, testing is done by an outside laboratory. Imaging is done at a regional center that has agreed to accept our patients in their indigent care program. Pharmaceutical assistance will be offered through use of generic, low cost medications or through Patient Assistance Program sponsored by pharmaceutical companies.

Applicants must complete an application in person by scheduled appointment.
If you are coming from the south or north of White County, take US-129 to the town square and locate the clinic on the west side next to Lanier Therapy in Motion.
If you are coming from the east or west of White County, take US-115 to the town square and locate the clinic on the west side next to Lanier Therapy in Motion.
There are parking available in front of or behind the building. Handicapped access is located in the front of the building as designated.
